Well the orange juice gets mixed into batches from multiple sources, so pinpointing the exact origin could be tricky. Also, it was only recently banned in the last few years.

According to this article that wiki cites the results of testing were consistent.
Also don't forget that far less than safe levels are required to do harm when multiple toxins produce a synergistic effect. I can't find the article, it was posted here previously, but the main point was that anywhere from as little to 1/25 to 1/100 of the "safe" dosage level was needed to produce harmful effects in humans.
*edit found at least one article that specifically talks about the synergistic effects of Carbendazim. Specifically it causes liver failure. Now let's say someone takes some tylenol and washes it down with some OJ...
